Jay was born on November 13, 1960 and passed away on Sunday, June 30, 2019.

Jay was a resident of Raymond, New Hampshire at the time of passing.

He graduated from Tewksbury High School, Class of 1978.

There are no calling hours. A Memorial Mass will be celebrated on Wednesday, July 3, 2019 at 10:00 AM at Our Lady of Mercy Catholic Church, 16 Baboosic Lake Rd., Merrimack, NH.

In lieu of flowers, donations may be made in his memory to either the American Diabetes Association , PO Box 15829, Arlington, VA 22215 or to Autism Speaks, Inc., 1 East 33rd St., 4th Floor, New York, NY 10016.
